Closing the Loop DAF Form
The purpose of this form is to close the loop on any change(s) you made in your respective area(s)/program(s) based on DAF data. The form asks what type of change(s) did you make based on the data you reviewed, when the changes went into effect, and what you learned from the change(s). Please use this form if you have already implemented a change(s) based on DAF data and are documenting the impact of the change(s) in your respective area(s)/program(s).
